Ordering
Please e-mail sales@atfirstdiagnostic.com or fax your purchase order to First Diagnostic LLC. The information sent should include the product catalog number, product description, quantity and price. We will send you an e-mail or fax confirming your purchase order along with an estimated ship date.
If your business does not use a purchase order, you may fill out the Sales Order Form on this web site and fax it to the attention of Customer Service at (516) 908-4161.
Terms of Payment
AT First Diagnostic the first order to be paid in advance or Irrevocable Letter of Credit drawn upon a US Bank. Credit terms are available at the discretion of First Diagnostic LLC to US customers only. To obtain credit, please complete and return the Credit Application by fax for review and approval.
- Checks and money orders should be made payable to: First Diagnostic, LLC. US bank checks only please. Foreign checks will not be accepted.
- For credit card payments, please complete and return the Payment Authorization Form to enable us to process your order 3-5 days.
- Prepayments by wire transfer can request the following bank info here wire@atfirstdiagnostic.com
Lead Time: 14 -45 days depending on PO. (special packing materials extra)
Shipping Information
First Diagnostic will ship orders using the customer's preferred carrier. If the carrier is not specified, we will use the most cost effective and timely service available. Please include the shipping address along with a contact name and phone number on the order. UPS, FedEx, DHL and most other freight companies will not deliver to P.O. Box addresses; a street address is needed.
Return Policy
Our goal is complete customer satisfaction, and we will address all customer concerns promptly. A Return Goods Authorization (RGA) number must be obtained from First Diagnostic prior to returning any product. Refunds or credits are given at the discretion of First Diagnostic. Authorized returns made at the customer’s request, and not resulting from action initiated by AT First Diagnostic are subject to a 30% restocking fee. First Diagnostic is not responsible for products that have expired, were used improperly, or were stored incorrectly.
